  • COMPENSATION:      $ 120,000 annually + Comprehensive Benefits Plan

POSITION SUMMARY:          

The Nurse Practitioner is responsible for delivering comprehensive primary health care services with a strong emphasis on health promotion, disease prevention, and community engagement. This role includes direct client care, program planning, and health
education. The Nurse Practitioner will support the health and well-being of newcomers and immigrant populations, in alignment with funding received from the Ministry of Health.

QUALIFICATIONS AND EXPERIENCE:

  • Nurse Practitioner Certification required
  • Master of Nursing or Master of Science in Nursing is considered an asset
  • Current registration with the College of Nurses of Ontario in good standing
  • Membership in the Registered Nurses’ Association of Ontario (RNAO) with professional liability protection
  • Minimum of 3–5 years of experience in community health or primary care settings
  • Demonstrated experience working with diverse cultural and linguistic populations
  • Strong leadership, organizational, and interpersonal skills
  • Excellent oral and written communication skills
  • Proficiency in Microsoft Office Suite (Word, Excel, Outlook, PowerPoint)
  • Flexible availability to accommodate service demands

  • Fluency in a second language is considered an asset

  • Valid Level 3 Vulnerable Sector Police Clearance, renewed every three years

AREAS OF RESPONSIBILITY:

  • Conduct comprehensive health assessments and physical examinations

  • Develop individualized health promotion and disease prevention strategies based on client needs

  • Deliver on-site health education, counselling, and wellness clinics for newcomers and immigrants

  • Provide client support through in-person visits, telephone consultations, and specialized programs

  • Coordinate care with other healthcare providers and community agencies

  • Participate in program development, implementation, and evaluation activities

  • Supervise, support, and evaluate volunteers assisting with health programs

  • Maintain accurate and timely documentation, including monthly reports and statistical submissions in accordance with Ministry of Health and MCC guidelines

  • Monitor client outcomes and ensure appropriate follow-up care is provided

  • Liaise with community organizations to strengthen referral networks and enhance program delivery

  • Identify and report emerging health trends and community needs to the Ministry of Health and MCC leadership

  • Promote and uphold a safe, inclusive, and respectful workplace in accordance with Occupational Health and Safety legislation

  • Maintain collaborative relationships with physician partners as per Nurse Practitioner Standards of Practice

  • Provide in-home or temporary site care, including visits to clients in quarantine, as required

  • Attend relevant workshops, conferences, and training sessions for ongoing professional development
